John Gantons Countryside Care Center is seeking a full time, Long Term Care, Social Worker for our long term residents. This position may also assist with resident admissions from time to time. Pay is negotiable based on experience.

The primary purpose of your position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations governing CCC, and as may be directed by the Administrator to ensure that the emotional and social needs of the residents are met and maintained and that the highest degree of quality care can be provided to our residents at all times.

  • Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of CCC.
  • Develop comprehensive assessments of the social service needs of each resident.
  • Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
  • Participate in community planning related to the interests of CCC and the services and needs of the residents and families.
  • Participate in discharge planning, development and implementation of social service care plans and resident assessments.
  • Interview residents/families to obtain social history.
  • Perform administrative duties such as completing medical forms, reports, evaluations, charting, etc., as necessary.
  • Involve resident/family in planning social service programs when possible.
  • Assist in arranging transportation of residents to other facilities when necessary.
  • Refer residents/families to appropriate social services that are not provided by CCC.
  • Provide resident/families information on Medicare/Medicaid, and other financial assistance that may be available to the resident.
  • Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other departments when necessary.
  • Develop and implement a plan of correction for social service deficiencies noted during survey, if applicable.
  • Assist the Quality Assurance Committee in developing and implementing appropriate plans of action to identify and correct deficiencies.
  • Participate in CCC surveys.
  • Write clear, descriptive, progress notes of services provided and the resident’s response to services.
  • Assist in the scheduling of care plans and assessments to be presented and discussed at each meeting.
  • Review and revise care plans and assessments as necessary.
  • Review complaints and grievances made by residents and make a oral/written report to the Administrator indicating what action(s) were taken to resolve the complaint or grievance.
  • Provide residents with information concerning resident rights, living wills, etc.
  • Participate in resident council meetings.
  • Maintain timely and accurate resident reports as required by federal, state, and local regulatory agencies and third party payers.
  • Perform all duties in an effective, timely, and professional manner.
  • Be responsible for the safety of residents’ under your care.
  • Comply with all facility policies and procedures.
  • Accept assigned duties in a cooperative manner.
  • Perform other related duties as directed by the supervisor.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.
  • Come to work in neat, clean attire and consistently present an appropriate professional appearance.
  • Consistently work cooperatively with residents, residents’ representatives, facility staff, physicians, consultants, and ancillary service providers.
  • Work in cooperation with other departments within the facility to provide a holistic approach to meeting resident needs.
