Fine dining, housekeeping, 24-hour emergency call system and an activity program geared towards an active lifestyle. Apartments feature a full and well appointed kitchen. Apartment sizes range from 637 sq ft to 996 sq ft.

Model open Monday – Friday 8 am – 5 pm and by appointment after hours.

A fun, and inviting activity program. From ballroom dancing classes to art lessons with acrylic paints, the possibilities are endless. All activities are open to all residents, regardless where they live on campus. We provide FREE on campus transportation!

Three meals daily. We encourage guests in our dining rooms. Meal tickets are available for purchase at any front desk. Our dining rooms offer open dining and are open from 7:00 am – 7:00 pm.

Specialized Alzheimer’s/Dementia Care

Countryside Care Center, our skilled nursing facility has a special wing with staff specifically trained to care for residents with Alzheimer’s and Dementia. We call it the Meadows.

This area includes:

29 Private and semi-private rooms

Medicare/Medicaid certified

Secured area to prevent wandering

Private dining and activity areas

Johnny O’s is a family owned company that opened to the public on February 1st, 2005 and is located in Countryside’s Atrium Professional Plaza (on the corner of Robinson and McCain) at the north end of the building.

The Countryside campus is equipped with it’s very own Atrium Professional Plaza. With several businesses available to serve not only our residents, but also the Jackson community, the plaza is a great asset to our campus. Orthopaedic Rehab Specialists, Brown’s Advanced Care Pharmacy, Ganton Home Care and Johnny O’s Fitness Center are all located right here on the corner of Robinson and McCain. Free on campus transportation is available for residents upon request. They just need to call the main Countryside number and ask for transportation.

Ganton Health and Rehabilitation Center

Ganton Health and Rehabilitation Center will be coming soon to Grass Lake! It is expected to offer skilled nursing as well as comprehensive therapy services with spacious living and gathering areas. The center will be separated into 49-beds for long-term care and 42-beds for rehabilitation.
