For those needing more care, we offer the Meadows. The Meadows is located in our Care Center and is a secure locked unit with 29 Medicare/Medicaid beds with private and semi-private rooms.
Johnny O’s Fitness Center
Johnny O’s is a family owned company that opened to the public on February 1st, 2005 and is located in Countryside’s Atrium Professional Plaza (on the corner of Robinson and McCain) at the north end of the building.
We offer circuit training equipment, stationary bikes, elliptical and treadmill machines. Johnny O’s also offers free-weights and state of the art weight machines to work target areas. Our restrooms have lockers and showers for your convenience.

Professional Plaza
The Countryside campus is equipped with it’s very own Atrium Professional Plaza. With several businesses available to serve not only our residents, but also the Jackson community, the plaza is a great asset to our campus. Orthopaedic Rehab Specialists, Brown’s Advanced Care Pharmacy, Ganton Home Care and Johnny O’s Fitness Center are all located right here on the corner of Robinson and McCain. Free on campus transportation is available for residents upon request. They just need to call the main Countryside number and ask for transportation.

Assisted Living
Our assisted living apartments offer great peace of mind, providing beautiful housing with several levels of assistance available to you. Apartments range from 350 to 996 square feet and are equipped with an available 24-hour emergency two-way call system that can be monitored on-site should you choose. Along with the exceptional living opportunities, our tenants can take advantage of the available on-site home health care services to assist them with medication administration, bathing, dressing and assistance with other activities to meet their needs 24-hours a day. Our extensive options, the services and amenities available on site often allow our tenants to remain comfortably in their apartment even as their physical needs change. If 24-hour nursing care becomes necessary at some point, our tenants can rest assured knowing they have priority placement in our award-winning Care Center.

Ganton Health and Rehabilitation Center
Ganton Health and Rehabilitation Center will be coming soon to Grass Lake! It is expected to offer skilled nursing as well as comprehensive therapy services with spacious living and gathering areas. The center will be separated into 49-beds for long-term care and 42-beds for rehabilitation.
Independent Homes for Purchase
For those looking to purchase a home, Terrace Hills Condominiums are for you. With main level laundry, spacious kitchens, and many upgrades available, the possibilities are endless. Terrace Hills Condominiums are close to many area businesses and only minutes away from major highways. We have attached condos and stand alone condos available at this time. We even have new construction underway!
Residents of Terrace Hills Condominiums are able to partake in many additional services, such as the grocery service ($4 handling fee), housekeeping, homecare, use of dining rooms and other Countryside common areas, Country Cafe deli, on-site professional services including a pharmacy, rehab company and low-impact fitness club. Residents also receive priority placement in Countryside’s other levels of living.

To complete our extensive array of services, we feature an award-winning, skilled Medicare/Medicaid certified nursing center. This exceptional residence features private and semi-private rooms staffed 24-hours by a wonderfully supportive and highly trained staff. Care includes; on-site physical, occupational, speech and massage therapy, with the availability of optical, dental and podiatry services. We also have a registered dietitian to ensure nutritional requirements. To further enhance life for our residents, we have a fun and highly energetic activity staff that creates innovative activities and programs. Our lovable resident pets also add extra comfort and companionship.
Independent Living for Rent
Our Villas are maintenance-free condominium-style homes in a neighborhood setting, with no cash investment or endowment fee! Choose from 900 to 2,000 square-foot, ranch-style, all-on-one-floor designs with attached garages and carports. Homes feature spacious kitchens with appliances, central air, high-efficiency air purifiers, screened porches or decks, and more. For your convenience, housekeeping and grocery delivery is available. You could also take advantage of the many outings on the monthly calendar. From casino trips to grocery or dinner outings, there is never a dull moment. Community members also receive priority placement in our award-winning Assisted Living or Care Center, if needed in the future.
